Перспективи нарощування фінансових ресурсів закладів охорони здоров'я в умовах війни та поствоєнного відновлення України

Abstract

У статті розкриті особливості формування фінансових ресурсів системи охорони здоров'я в умовах воєнного стану, охарактеризовані основні стратегії вирішення проблеми дефіциту фінансування галузі та ймовірність їхньої реалізації. Обґрунтовано позитивні наслідки активізації добровільного медичного страхування, впровадження співоплат при отриманні медичних послуг як форми легалізації тіньових платежів у закладах охорони здоров'я. Окрему увагу приділено розкриттю переваг спеціалізованого фасиліті-менеджменту, що дасть можливість досягти економії експлуатаційних витрат закладів охорони здоров'я без негативного впливу на якісні параметри надання медичних послуг. Розглянуто перспективи нарощування фінансових ресурсів закладів охорони здоров'я від надання телемедичних послуг, залучення коштів накопичувальних фондів територіальних громад і благодійних фондів, розвитку державно-приватного партнерства, корпоративної соціальної відповідальності / The article reveals the formation peculiarities of health care system financial resources in the martial law conditions, characterizes the main strategies for solving the problem of the industry's funding deficit, and evaluates the possibilities of their implementation. These strategies assume, that the problem of health care financial resources deficit can be solved in the following ways: budget expenditures redistribute in favor of health care by refusing to other budget programs financing; attract additional resources for the health care system from raising taxes or targeted fiscal payments introducing; attract resources from international technical assistance programs, international and domestic donors; free up resources due to costs control in health care institutions. However, budgetary resources additional redistribution and the new fiscal payment's introduction for health care financing may have negative consequences for economy in war conditions and will be negatively perceived by society. Authors have been identified the positive consequences of voluntary health insurance activation and co-payment (medical deductible) introduction during medical services receiving as a form of shadow payments legalization in health care institutions. Particular attention is paid to disclosure of advantages of outsourcing using in health care institutions, which will make it possible to achieve the operating costs savings in health care institutions without a negative impact on the quality parameters of medical services provision. A facility management in the health care institutions should provide for the auxiliary non-medical personnel recruitment, the financial resources saving due to the wholesale purchase of consumables, food products, and construction materials. It is substantiated that the mandatory health insurance introduction is the most rational in modern conditions with the distribution of responsibility for the payment of insurance premiums between the state, workers and employers. Prospects for increasing the health care institutions financial resources from the telemedical services provision, attracting resources from local community savings funds and charitable funds, public-private partnerships developing, and corporate social responsibility were also considered.
